Two more ambulances were in Cork on Monday that could have been sent to the aid of an injured toddler, who later died from his injuries.
Lifeline, a private ambulance company says it was already working for the HSE on the day 2-year-old Vakaris Martinaitis fell from an upstairs window in Midleton.
A formal review has now been launched by the HSE, after it was revealed that a third ambulance, belonging to the HSE, was actually available just 18 minutes away.
An emergency call was made by the boy's neighbours but they were told no ambulance was available to be sent. The boy died in hospital two days later.
