A report into the death of Aoife Johnston at University Hospital Limerick has found a series of failings in the facility's handling of her care.

The 16-year-old died after contracting sepsis after waiting on a chair for treatment in December 2022.

The report by former Chief Justice Frank Clarke said the circumstances that led to the teenager's death were almost certainly avoidable.

It described UHL as understaffed and said overcrowding was more severe than it should have been.

Doctors and nurses at the facility were found to be unaware of the risk of sepsis which Ms Johnston was suffering from.

There are 17 recommendations as part of the review aimed at improving how patients are dealt with in the UHL ED.

They include improving the electronic system and calling for a review into why overcrowding is such a problem at the facility.

In response, the head of the HSE said the report gives an opportunity for learning and accountability.

An inquest, held in April, returned a verdict of medical misadventure into the death of Aoife Johnston.
