A spoon with drug residue, a used hypodermic needle and two empty bottles of champagne were discovered in the hotel room where Glee star Cory Monteith was found dead, officials have said.

A coroner's final report confirmed initial findings that the Canadian actor died from using intravenous heroin, combined with alcohol.

Monteith (31) had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs.

Monteith was found dead in a Vancouver hotel room on July 13th - the day he was due to check out.

Hotel staff found him collapsed on the floor after entering his room.

Coroner Claire Thompson said: "It appeared (he) had been dead for several hours."

"I classify this death as accidental."

In August, Monteith's girlfriend and Glee co-star Lea Michele paid an emotional tribute to him.

The pair played an off-again, on-again couple in the TV show and dated off-screen for about a year.

Michele tearfully addressed the audience at the Teen Choice Awards as she took to the stage to accept a gong for her role on the hit show.

Wearing a necklace that spelled out the name "Cory", she said: "I just wanted to be here today to personally thank all of you and tell everyone out there how much all of your love and support has meant to me over these very past difficult few weeks."

She went on to dedicate the award to Monteith.
