The Health Service Executive (HSE) is to review the case of Cork man Michael Greaney, who died alongside his wife on Sunday less than two months after leaving the Central Mental Hospital in Dublin.
Meanwhile the couple's eldest daughter remains in a serious but stable condition in hospital after being injured in the incident in Cobh.
Mr Greaney (53) was found dead alongside his wife Valerie (49) at their home in O'Neils Place in Cobh on Sunday afternoon.
It has now emerged the physiotherapist had been involved in litigation with three different banks and feared he could lose the family home over his debts.
He had battled mental health issues and had spent some time at the Central Mental Hospital in Dublin.
However, a decision was made at the end of October to release him back to the family home in Cobh.
According to the Irish Independent, a full scale review of his treatment will now be carried out.
Post mortems were carried out on the couple yesterday. They reveled Mr Greaney died from a single stab wound that pierced his heart - while his wife suffered several stab wounds.
The couple's eldest daughter was also seriously injured and remains in hospital.
