A number of hospitals across the country reportedly placed the bodies of dead babies into coffins with deceased adults, according to a new report from RTE.
They are reporting that a number of “tandem burials” were conducted across a number of hospitals.
A letter obtained by the broadcaster states that they are unsure if the families of both parties were aware of the practices. Secretary general of the Department of Health, Dr Ambrose McLoughlin, was briefed about the issue in the letter from Director General of the HSE Tony O' Brien which the broadcaster obtained.
It apparently occurred in “exceptional circumstances” when there were no other options for burial.
The letter says: "Our understanding is that the remains would have been placed with adult remains and that ideally the awareness and understanding of both families involved would have been sought, though this is not guaranteed."
RTE reports the document states that hospitals have confirmed the practice, particularly mortuary staff.
The HSE has appointed a liaison to deal with potential inquiries into the matter.
