A report into the death of a prisoner in Cloverhill Courthouse has found that organs of the prison system failed to communicate with each other, failed to adhere to standard work practices and did not keep adequate records.

Shane Rogers was found hanging in his cell on the 22nd of December 2011 after admitting killing Crossmaglen GAA player James Hughes.

The report has been published by the Minister for Justice.
