Sligo Regional Hospital has issued an apology to the husband and family of Sally Rowlette, who died in their care last year.

The 36-year-old died after giving birth to her fourth child in February 2013.

The jury at the inquest into Mrs Rowlette's death returned a verdict of death by medical misadventure yesterday.

In a statement this evening Saolta University Health Care Group, which runs the hospital, offered their condolences for the events which contributed to her "tragic" death.

“Sligo Regional Hospital-Saolta University Health Care Group wish to apologise to Mrs Sally Rowlette’s husband and her family for the events related to her care that contributed to her tragic death and to take this opportunity to again offer them our condolences,” the statement said.

The group said they wanted to reassure their patients that the delivery of safe patient care continues to be a priority at Sligo Regional. The statement listed several areas of patient care where improvements have taken place in recent years, “specifically relating to maternity and ICU services.”

Sally Rowlette's husband Sean spoke to Newstalk Lunchtime today and he called on Leo Varadkar to meet him to discuss system of care: "I'd like to tell him my story and the pain we have gone through for the last two years. I want to explain that these things has to be changed. Our family has been torn apart."

"I want him to hear it first hand."
