Crumlin Children's Hospital has apologised for an administrative error at an inquest into the death of a teenager at a Kodaline concert.
17-year-old Ciara Lawlor from Kilkenny collapsed at the Marlay Park gig in Dublin in July 2016.
A pathologist today told Dublin Coroner’s court that she had a rare genetic heart condition that can cause sudden death.
The court heard that she had been receiving treatment for heart palpitations for a number of years - including an operation and different medicines – however, the symptoms continued and her condition remained undiagnosed.
Meanwhile, an admin error meant an appointment for an important follow-up test was never sent out.
The coroner ruled that she died of natural causes.
One of Ciara's friends, Aoife Murphy, was visibly upset giving evidence at the inquest.
She said they got the bus to Dublin on 9th July 2016, met up with some other friends and went to Marlay Park for the Kodaline gig.
The inquest heard how Ciara seemed fine on the day and made no complaints about how she was feeling.
But 3 or 4 songs from the end, she collapsed and was then transferred to hospital where she later died.
There was hardly any alcohol in her system and no drugs.
Her mum Eimear spoke after the coroner delivered a verdict of death from natural causes.
“She identified a need for improvement in communications in hospitals and also Crumlin Hospital has acknowledged and apologised to us for system failures and have indicated to the coroner that they are going to be addressing these,” she said.
The coroner also recommended that the hospital should give parents a written information document about the medicine their child is prescribed.
