Earlier this week Mental Health Reform launched the first of two national surveys into people's experiences of mental health services provided by the HSE.

Susan spoke to Elaine Clear and her son Rory about their son and brother Dan Hogan who died by suicide, aged 17.

The family say Dan was failed by "mental health services".
